Requirements:
MD or DO degree from an accredited medical school.
Completed residency in Family or Internal Medicine accredited by the Accreditation Council for Graduate Medical Education
Board certified or board eligible in Family or Internal Medicine
Active and unrestricted medical license to practice in Arizona.
Current DEA registration for prescribing controlled substances.
Prior experience in a family medicine setting preferred.
